Expert Geriatric Care Management – Navigate a Dementia Diagnosis with Confidence
Have you, your spouse, partner, or a parent been given a diagnosis of dementia or Alzheimer’s?
Are you facing a mountain of stressors and unknowns, overwhelmed by what’s next?
Do you wish someone could guide you through the process of understanding the diagnosis, how to navigate the geriatric care system, and which resources will actually help your family?
You’ve Come to the Right Place
While my practice is dedicated exclusively to dementia care, I manage the whole person. This means my services directly include overseeing and coordinating care for any other chronic health conditions your loved one has alongside dementia—such as diabetes, heart disease, or mobility challenges.
Dementia is a progressive disease, and families often find themselves in a "gap" where the current level of care—or their current plan—is no longer enough.
Whether you are navigating the shock of a recent diagnosis or finding that your loved one’s needs are outpacing the support you currently have in place, you are likely facing a mountain of questions and unknowns.
These concerns can become entirely all-consuming as you’re left to wonder:
What does the diagnosis actually mean and how quickly will it progress?
Are there ways for my loved one to still live independently?
If I can’t be the caregiver, who can help?
When should home care and memory care be considered, and what should I be looking for in terms of assistance and accommodations?
How can I advocate for my loved one and make the most out of doctors’ visits within the ever-complex healthcare system?
My expertise is the management of geriatric care—guiding families in situations just like yours to find answers and the right support.

I Know How Overwhelmed You Feel Right Now
Dementia care is complex, and general information rarely suffices to help families navigate the system. Without a proactive strategy, they often find themselves reacting to crises.
I’m here to help so you don’t have to navigate this alone.
My role as a care manager is to cut through the noise with clinical insight on seniors and strategic planning customized to your loved one’s needs. I partner with you to anticipate each stage, putting the right supports in place before they become crises.
Let My Expertise Work to Your Advantage
Between my work with the Alzheimer’s Association and the University of Illinois’ Memory and Aging Clinic, I have worked alongside specialists in many capacities—including Clinical Liaison, Care Consultant, Care Navigator, Early Stage Support Group Facilitator, and Expert Panelist—developing an extensive understanding of Alzheimer’s and related dementias.
Having supported hundreds of families as they adjust to life with a dementia diagnosis, I know one thing for certain: no two dementia journeys are exactly alike.
My work is built on this understanding, allowing me to move beyond one-size-fits-all advice and provide the deeply tailored, compassionate care planning your family deserves.
It’s never too early (or too late) to reach out to a geriatric care manager who specializes in dementia support.
Dementia and Alzheimer's Care Management: From the Unknown to the Manageable

I treat all information shared during our consultations—including medical history and family dynamics—with complete confidentiality. As a licensed professional, I bring a clinical standard of privacy to our work, ensuring that our time together remains a safe, trusted space for you to address your family’s unique challenges.
Dementia care management services are well-suited for families whose loved one is living with any type of dementia, including:

Finding Answers and Peace of Mind by Turning Overwhelming Questions into a Practical Care Plan
Working with me includes three main steps:
Understanding your needs: We begin with an intake process to gain an understanding of where your family is right now, what your goals are, and what your loved one's overall health, lifestyle, and wishes are.
Strategy: Based on this, I give you an idea of what’s ahead and identify which resources will be the most helpful now and in the future for your specific situation.
Care Planning: Together, we craft a solution-focused approach that honors your family’s unique lifestyle, starting exactly where you are today.
If you are currently in the middle of a crisis, we can focus on immediate stabilization, safety, and navigating the next right step.
Whether you are looking to prevent a crisis or manage one that is already unfolding, dementia care management provides the expert roadmap you need to regain your footing, preserve your loved one’s dignity, and protect your peace of mind.
